2nd ANNUAL GOLF TOURNAMENT

Please come join us for an afternoon of golf, prizes, food and fun! The Randy Cappiello Foundation, formed in loving memory of Randy Cappiello who lost her battle to lung cancer, is hosting a golf tournament to support research for the prevention, detection and cure of lung cancer. Lunch is available and registration begins at 12:30pm, with a shotgun start at 1:45pm. The tournament will be in the form of a best ball scramble and prizes will be awarded for the winning male team, female team and mixed team, as well as the longest drive and closest to the pin. A hole in one wins $10,000! If you are not a golfer, please consider coming to the dinner banquet at 7pm.

There are many other ways to help in addition to your participation, including:

• Ask your employer to be a sponsor of the tournament or of a tee
• Sponsor a tee or be a tournament booster yourself
• Donate a prize (a gift certificate, golf merchandise, sports tickets, you name it!)

We hope to make this an enjoyable and worthwhile event. All the profit from the tournament will go to fund Lung Cancer research. Lung Cancer continues to be the number one cause of cancer deaths. Your participation in this tournament will help get us one step closer to a cure and it is sure to be a great day.

Please arrive at 12:20am for registration

Cost: $150 per golfer
Includes Greens fees, carts, contests, Lunch and dinner

Or $60 for dinner only (begins at 7pm)

Levels of Sponsorship

The Randy Cappiello Foundation for the prevention detection and cure of Lung Cancer is proud to present the different levels of sponsorship available to you and/or your company. All profit from the tournament will go to Lung Cancer Research.

Tournament Sponsor $7,000
The Tournament Sponsor will have their banner hung at the banquet following the tournament, a booth at the registration area (if desired) as well as a sign at the first hole. Additionally, the Tournament sponsor will receive a free foursome.

Dinner Sponsor $4,000
The Dinner sponsor will have their company banner hung at banquet as well as their sign at the 10th hole. Additionally, the Dinner sponsor will receive a free foursome

Cart Sponsor $2,500
The Cart sponsor will have their name on every Golf Cart as well as a sign on the 18th hole.

Lunch Sponsor $1,000
Your Logo and company information can be added to lunches distributed at the start of the tournament.

Prize Sponsor $300
For closest to the pin or longest drive holes, your company will have a sign at the tee box noting you are the prize sponsor as well as recognition of your organization during the banquette as the prize is awarded.

Hole Sponsor $150
A large sign located at the tee box to indicate your sponsorship. A great way to advertise for your business or a way to honor a loved one.

Purchase a Tent Sign $50
A small sign will be displayed inside or around the tent during registration and dinner.

Individual Player Fee $ 150

Foursome Fee $600

Banquet only (begins at 7pm) $60
